How often does a flow run?

As often as you need: you set the schedule for each flow and can pause it any time.

Will it place real orders or create real accounts?

It does exactly what you recorded. To avoid real orders, record the path up to the payment step, or use a test account.

Which browser runs the flow?

A real browser, the same kind your visitors use, with a desktop window by default. The window size can be changed for each flow.
